Welcome to CUSD here you will find the necessary forms to enroll your child in 6-12th grade Newcomer Program. Please make sure that you have the done the following *withdraw your child from previous school and have the most recent report card. Have their immunization records up to date, for 7th-12th make sure the student has had their TDAP/whopping cough vaccination. *The following forms should only be filled out in a necessary basis, *caregiver affidavit (only if you are not the legal guardian, *for foster youths please provide the placement letters, * suspension and expulsion placement letters if applicable.
