DOCUMENTS REQUIRED FOR REGISTRATION
(For ALL new or returning students to CUSD)

- Photo Identification of the Parent or Legal Guardian (REQUIRED)
- Proof of Residence within the school boundaries (ONE OF THE FOLLOWING):
Must be in the name of the Parent/Guardian registering the student
- Property tax payments receipts
- Current electricity, gas, and/or water statement
- Paystubs
- Voter registration
- Correspondence from a government agency
- Affidavit of Residence (CUSD Form only), executed by the parent/legal guardian and the owner/renter of property, accompanied by Current Utility Bill (Electricity, Gas, and/or Water) in the owner/renter's name.
- Proof of Age (ONE OF THE FOLLOWING)
- Copy of Birth Certificate (Original required for TK through 1st grade)
- Baptism certificate with official seal
- Foster placement letter to show guardianship
- Guardianship/Court documents
- Health Insurance Verification
- Proof of Immunizations (REQUIRED)
- Polio
- Diphtheria, Tetanus, and Pertussis
- Measles, Mumps, Rubella (MMR)
- Hepatitis B
- Varicella
- TB Testing (Mantoux) with results (1st grade only)
- Tdap (Whooping Cough) 7th - 12th Grade
- Oral Health Assessment results required for first time Transitional Kinder/Kindergarten & 1st grade students--Recent Official Health Exam Report required for Transitional Kinder/Kindergarten & 1st grade students (These documents must be signed by the Doctor and stamped with the doctor's official office stamp.)
- Scholastic Records that include: (REQUIRED WHERE APPLICABLE)
- Unofficial Transcripts with check out grades (9th - 12th grade)
- Check out papers/PAR from previous school attended and last report card(1st-12th grade)
- CELDT Results (EL students)
- Submission of any Standardized Test Results administered by former school
- IEP and Triennial Report (Special Education students)
- Section 504 Records(Student with Section 504 plan)
- GATE Assessment records
- Completed, signed and dated CUSD Student Registration Forms, Page 1 - 5.
Please write legibly and use only blue or black ink. Do not use a pencil to complete these forms.
NOTE: The appropriate documents must be submitted to complete the registration process. The parent or legal guardian may submit a duplicate or photocopy of any of the above documents after presenting the original documents to the enrollment staff in NSOC.
